Lampmaster: Lamp Recycling Box Program

Instructions

1. Remove the enclosed FedEx PRP Shipping Label and attach it to the pail. Keep the FedEx shipper receipt for your records. You will need the number from the receipt to give to FedEx when you schedule the pickup. If you do NOT have a shipping label, please contact USA Lamp and Ballast at 888.676.0044.

2. Shape box and tape ALL flaps on one end of the box.

3. Place the containment bag inside the box.

4. Fill in Generator Information and Accumulation Start Date.

5. Begin to fill the box with spent mercury-containing bulbs. Do NOT break or crush lamps.

6. DO NOT put shielded lamps in the same box with regular lamps and DO NOT put materials other than lamps in the lamp boxes.

7. After the box is full, fold over the end of the bag and tape ALL flaps on open end of the box.

8. Complete Accumulation End Date and Enter the Quantity of Bulbs.

9. Please be sure that the Red FedEx PRP Shipping Label is attached to the box. No additional labels or markings are required.

10. To schedule pick-up of the pail, call 1-888-777-6040 for the FedEx Ground Package Returns Program. The FedEx Representative will request the number from the shipping receipt - 1196802.

11. A Certificate of Recycling is available upon the completion of the recycling process. An e-mail address or fax number is required to receive the Recycling Certificate.
